How to Access Services & Schedule an Appointment

• Call to schedule an assessment across all locations in Montgomery, St. Peter, and New Ulm.

Things you will need at the time of Scheduling

• Insurance Information- member number, group number; If covered by parent their name and address and date of birth
• Date of birth, address, phone number, email, and emergency contact
• Substance use disorder intakes: social security number, release of information, probation officer, attorney, primary care provider, drug court
