Meet the Team

Case Managers

Case managers help people identify, access and navigate supports and services. Case managers are responsible for providing the information a person needs to make informed choices about supports and services. This includes social, physical and mental health, educational, vocational, and financial services. To work as an effective team, case managers connect people with community resources.

Therapists

A therapist is trained to help people address, cope with, and reduce mental, emotional, or behavioral issues. They provide support, guidance, and treatment to improve psychological well-being and mental health

Addie Evans

Addie EvansHi there!

As a born-and-raised Midwesterner, my heart has always been rooted in our heartland communities. Growing up in  Kansas City taught me firsthand about the unique healthcare challenges we face here in the Midwest, and it's shaped everything about how I approach healthcare advocacy.

My journey took me to Fairhaven College of Western Washington University, where I studied Intersectional Feminist Health Practices, and then to positions with Planned Parenthood in Colorado and Oregon. But the Midwest kept calling me home, and I'm grateful to be back serving our community. Over my six years in reproductive and mental healthcare, I've focused on making healthcare more accessible and stigma-free, especially for folks who have traditionally faced barriers to care.

I understand the distinct needs of our rural communities because I've lived them. Whether you're coming from a small town or the city, I'm here to help navigate healthcare systems and advocate for your needs. My goal is to ensure everyone in our community has access to compassionate, understanding care that respects their background and experiences.

I'm committed to bringing both my professional expertise and my deep connection to Midwest values to support you. I look forward to working together to address your healthcare needs with the care and attention you deserve.
